Who provides this Service? The Service is provided through a partnership between Age Concern (Lindsey) and Louth & District Hospice. The Home Support Service (Palliative Care) is not a nursing or personal care service. However, all the Home Support Assistants receive specialist training in palliative care needs. Can I Use the Service? The Home Support Service is for people with palliative care needs. People with palliative care needs are those who have a disease that cannot be cured. Some of these people may be in the last few months of their lives and may have complex care needs. Just coping with everyday life can be a real struggle, so practical help and support is essential during this time.
What will the Service provide? The Service is not a nursing or personal care service, but will provide:
When is the Service available ? The sitting service will be offered during the day or night, seven days a week, subject to availability. All the other services will be provided between 9am - 5pm Monday - Friday. How Much Does the Service Cost?All the services are free at the point of service for any person with palliative care needs. How do people get the Service?Only District and Community Nurses can refer people to the service. Please contact your GP surgery if you are not sure which nurse you should talk to. What happens after a referral is made? An Age Concern (Lindsey) Scheme Manager will undertake an assessment of the individual’s needs. Once the service has been agreed, Age Concern (Lindsey) will leave a pocket file in the person’s home with contact numbers and details of the service to be provided. In what areas is the Service available? Widely available throughout East Lindsey and the Lincolnshire Wolds, some of the towns covered are: Alford, Binbrook, Caistor, Chapel St Leonards, Holton le Clay, Horncastle, Louth, Mablethorpe, Market Rasen, Spilsby, Sutton on Sea, Tealby, Woodhall Spa, Wragby How to make a referral Please ask your District / Community Nurse to contact Age Concern (Lindsey) on 01507 524798.
